Keto, carnivore, Mediterranean: comparing diet styles without the cult

Diets go viral; busy lives need sustainable fuel. Here is a clinician-educator view of four common patterns—without telling you which tribe to join.

Mediterranean-style

Emphasizes olive oil, fish, plants, legumes, moderate wine (optional). Strongest long-term data for cardiovascular and metabolic risk in trials such as PREDIMED. Fits many families and firehouse meals with minor swaps.

Low-carbohydrate / ketogenic

Very low carb pushes ketone production. Can improve triglycerides and HbA1c short term in type 2 diabetes under supervision (ADA standards of care). Challenges: shift-friendly snacks, fiber intake, lipid monitoring, and rare but serious events in certain conditions—requires clinician oversight.

Carnivore (all-animal)

Essentially eliminates plants. Anecdotes abound; controlled long-term data are sparse. Risk: low fiber, micronutrient gaps, social and firehouse practicality. We rarely recommend it as a default for people with physically demanding or irregular schedules.

Standard American "processed default"

Not a branded diet—but the hidden pattern (fast food, sweet drinks, ultra-processed snacks). Fixing this alone helps many members before any named diet.
